In order to get assistance from Loaves and Fishes, you must meet certain guidelines set by the United States Department of Agriculture. The form below outlines the maximum amount your household can make divided by the amount of people in your household.

Loaves and Fishes serve clients living within a geographic area that runs from Eddystone (Delaware County, Pa.) to Glenolden and from Tinicum Township up to Rutledge. This area is composed of the Interboro and Ridley School Districts.

Because the guidelines change year to year, we list the current form only for you to see if you qualify. You do not need to print out this form. When you come in to apply, we will provide this form. The requirements shown below are effective July 1, 2015 to June 30, 2016. The calculations are reset each year by the federal government.

Self Declaration of Need Form

In order to be eligible to register as a client, each applicant must bring with them the following information:

  • Current proof of address – a piece of current mail, such as a bill - PECO, phone, medical, etc. with the clients name and address. NOT a driver’s license.
  • Proof of birth - for each member of the household. For adults this CAN be a driver’s license or birth certificate. For children, their birth certificates OR in some cases their date of birth is on their individual medical cards.

Please call ahead of time to check the information you need to bring with you to register. First time clients need to come in 30 minutes before closing. If you can, please bring carry bags.
